A Survey on the Actual Status of Multifocal Soft Contact Lenses Prescription and Evaluation of their Fitting Status for Korean Opticians

Abstract
Purpose: This study investigated the actual state of multifocal contact lens (MFCL) prescription and evaluation of their fitting status by opticians in practice.

Methods: A survey on the actual state of MFCL prescriptions of 118 Korean opticians who had experience in prescribing MFCLs was conducted by employing social networking service (SNS) and an online community website for opticians from July 2017 to April 2019. Multiple responses for the questions were allowed.

Results: Among 118 respondents, 94.9% answered that they had prescribed MFCLs to over 40-year-old contact lens wearers. Moreover, MFCLs were also prescribed to adults under their 40s, with 30.5% in their 30s, 12.7% in their 20s, and 9.3% in their teens. Although the respondence rate was highest for presbyopia prescription, MFCLs were also prescribed to reduce accommodative anomalies, as well as the visual fatigue induced by near work. The optical zone design of all MFCLs released in Korea was a center-near design by the end of the survey period, and 74.6% of respondents knew the optical zone design of prescribed MFCLs exactly. The measurement of the pupil size was mostly performed using a measuring instrument; however, not a few case was conducted by a ruler or naked eye with relatively lower reliability. 83.1% of the respondents answered that they had performed the evaluation of lens fitting after prescribing MFCLs with 75.5% in the lens movement after blinking, 65.4% in the lens centration, 53.1% in the push-up test, 14.3% in the lateral gaze lag test and 10.2% in up-gaze lag test.

Conclusions: From the results, it appears that opticians are relatively aware of the prescription and fitting of MFCLs; however, there are some minor issues. Therefore, continuous education regarding the optical zone design of MFCLs and fitting evaluation is necessary for eye care practitioners in order to prescribe the most adequate design of MFCLs for contact lens wearers.
